Papers HRA usually asks for
HRA can accept an application with a name, an address if you have one, and a signature. They will still ask for proof later. Photograph papers in good light. Upload in the ACCESS HRA app. Do not mail originals. Do not email papers to Apply Anyway.
Common proofs
- Your photo ID
- Birth certificate for each child (shows you are the parent)
- Social Security card if they ask — we do not want that number
- Official lease with every name, the rent, and whether heat / gas / electric is in the rent
- Or a landlord letter that you pay rent, plus heat, gas, or electric bills
- Pay stubs (last 30 days) or a letter from your boss
- If you work for yourself: last 3 months of money in and costs — not a pay stub
- Bank statements. In New York, savings alone do not stop food stamps
- Child support you get or pay
- Unemployment, Social Security, SSDI, or SSI award letters
- College half time or more: school letter for this term (CUNY/SUNY can use LDSS-5172)
- Food stamps health skip: form FIA-1021w signed by a doctor or counselor
Use a letter template if you have no pay stubs or no lease. Then upload the signed letter on the City app.
Official lists: HRA SNAP document guide and Cash Assistance FIA-1227A / W-119D. The worker can still ask for something else.
